The referral form issued by your Dentist or Doctor.

Your Medicare card, Pension Card, Healthcare Card, DVA Card.

What do need to tell us about

Do you have a pacemaker inserted?

Do you have any allergy to plastics?

Do you have any metal implants?

Do you have a Cochlear implant?

Do you have any device that is emitting any form of radio-wave?

Are you pregnant or could be?

Remove any jewellery from you head or neck.

Turn off mobile phones.

Turn off any other pager devices.

Turn off any radio transmitters

·                All procedures in our clinic attract a Medicare Rebate. Only with a authorised referral.

·                 If not eligible for bulk billing, some co-payment will be required on the day.

·                DMDI does not issue invoices.

·                Payment can be made using Visa, Master-card or EFTPOS.

·                All health care card holders, pensioners and DVA patients will be bulk billed.

·                You will leave with a CD to give your referring Dentist or Doctor;  or a copy can be issued for your own records, if required.

·                On many occasions we will send the report prepared by our Radiologist to your referrer with a CD.

·                Please note we do not print film.

• You will need to let us know how you wish to be rebated.
• All procedures are subject to an appointment unless we have time for walk in patients.
• This practice is accredited by the Australian Government.
• All devices used are included on the register of therapeutic goods in Australia.
• This practice is registered with the Victorian Government. (Department of Health Services).
